Aletheia is in private beta

A macOS app that turns video-call therapy sessions into progress notes on your Mac, without sending patient data anywhere. Free to licensed clinicians who apply.

Aletheia is a native macOS app for clinicians who see patients over video. It records the session, transcribes it, drafts a progress note, and lets you search and ask questions across a patient's history. All of that happens on the Mac, and no patient data is sent to a server. The app makes only a few fixed network requests, through a separate helper: an update check, a signed list of revoked license keys, and the AI models you choose to download. None of them carries patient data.

Why on-device

Session audio is about as sensitive as data gets. The usual answer is a cloud service with a business associate agreement. We wanted to see how far you could get without one. On a recent Apple Silicon Mac the answer is: far enough to write the note. Intel Macs work too, with smaller, slower models.

  • Transcription runs locally with whisper.cpp on Metal. Your microphone and the call audio are captured as separate tracks, so the transcript knows who said what.
  • Note drafting runs a local language model, constrained by a grammar, so the output always matches the format you chose: narrative, SOAP, DAP, BIRP or GIRP.
  • Models are sized to your hardware and downloaded in-app. Each one is pinned to a commit and verified by SHA-256.

The app has no accounts, no analytics and no telemetry. By default, raw audio is deleted once it has been transcribed.

Asking questions of the record

Ask answers questions about one session or about a patient's whole history. Answers about a patient's history say how much of the record they actually read, so you can tell a grounded answer from a partial one.

Global search uses SQLite FTS5 with sound-alike matching for names. Speech models misspell names, and a search that can't find "Katherine" when the transcript says "Catherine" is no use in practice.

Security posture

The app runs in the App Sandbox. It has an app lock and an audit log that records actions without recording patient content. The repository maps its safeguards to the HIPAA Security Rule. Aletheia is designed to support HIPAA safeguards; it is not HIPAA certified, and it doesn't replace your own policies and risk analysis. Released builds rely on macOS FileVault to encrypt your disk, and Aletheia doesn't back up your data, so keep your own encrypted backups.

Joining the beta

The beta is free and open to licensed clinicians, and to pre-licensed clinicians working under a licensed supervisor. Access is reviewed, so applying doesn't guarantee a key.

  1. Apply on the beta page. You tell us about your practice, read how Aletheia handles patient information, and tick each attestation.
  2. Confirm your email. The link works once and expires in 7 days.
  3. Sign. The agreement is generated from your answers and signed on the site. You can download your signed copy.
  4. Review. We check your application and your license.
  5. Get your key. If you're approved, we email a single-use link to your license key and the download.

A beta key lasts six months. About 30 days before it ends, we email you a renewed key. If it lapses, recording and AI keep working for a 14-day grace period, then pause. Your records stay yours either way: without a valid key you can still open, read, search, export and delete everything you've recorded.

The beta needs macOS 14 or later. We recommend Apple Silicon with 16 GB of memory, and you'll need about 3 to 8 GB of free disk space, including the models. The design documents, including encryption (an opt-in feature of developer builds) and the fine-tuning pipeline, are linked from the project page.

Feedback

Use Report a Bug… in the app or the feedback page to report a bug or request a feature. We confirm your email, review the report, and post it as a public issue in the project's GitHub repository. The email address you give us is never included in the issue. Don't include any client or patient information.
